Can you be a bit more specific? When this happens are you in an application, on the desktop?
Does this happen if the computer is just sitting there and you are not touching it or does it happen if you are doing something, e.g. switching to another open application with Alt + Tab, while you are typing an email or doing other stuff?
Try to give some examples.
On my laptop Jaws said something like that a few days ago when I wasn't doing anything. When I checked it was sitting on some blank screen, I didn't have any applications open, but it wasn't focused on the desktop, either. As soon as I pressed Windows Key + M to put focus on the desktop that stopped.

i loose focus every time i close any program and go back to the desktop with both windows 7 and 10. i have to mouse click the desktop to put focus back.

Yes. Any time I walk away from my computer, returning finds it has lost focus. Often when opening a program, I press enter on the program name, and JAWS reports that it has lost focus.

Whenever I first open Outlook I always have to shift tab in and out before
Jaws has focus and also, whilst typing or reading an e-mail, Jaws often
jumps out of the e-mail and I have to shift tab back into it.
I am using Jaws 18 on Windows 10 with Office 365.

I am so pleased you have mentioned this as I was going to buy a new
keyboard, thinking that my keyboard might be at fault.
Almost everytime I use a combination of keys like alt/tab or windows key/d
to get to the desktop, the computer behaves as though the alt or windows key
is still depressed - it isn't, but that is how it behaves. In fact it
happens for that little group of three keys, alt, windows key and the
control key, but it is the alt which is particularly nuisancesome.

Same with me here. I use the alt a lot because I have six languages stored
and have to cycle with alt-shift among them. Often alt especially, appears
depressed and prevents me from doing other things. I also recently had a
problem when taskbar somehow pops up and freezes jaws and perhaps, movement
of keys. Then, it either takes me an effort to get out of this, often by
quitting and restarting jaws, but I somehow often get to a page which
concerns screen magnification which I understand, is a narrator thing from
which I exit with some hard effort. Unlike with a previous posting, narrator
doesn't seem to start together with jaws, but comes in later, me being
unable to say what has gone wrong.

I haven't been following this too closely, but I've never had the problem.  One possible common denominator here may be the use of outlook.  I've never used it, and very seldom used any other MS email client, and have never had the problem.  Maybe that is a clue.

i think there is a setting to turn the announcement on or off. i turned it on because without it i had no way to know where focus is.
but i don't remember where this option is any more.
