the only thing I can think of is that it was a chrome update which set me on this path I noticed it about mid to late spring of this year so I suspect I’d have to roll back fairly far even if I could.

I'm having the same thought and NVDA updated this morning when I ran it to see how it behaved with Chrome and to this point, it has done so with no subsequent bad behavior.
