I'm running jaws18, windows10,
And when I'm in a program, for example, outlook, When I press alt+f4, I get placed back on the desktop.
I make sure, that the taskbar is clear.
By default, windows has internet browser, media player, windows explorer, etc., So, I did unpin these items from taskbar.

i don't know how to explain it. but windows m does not minimize all program screens. if you can see at all try it.
load a program that you can open a dialog within the program. then press windows m. focus will go to the desktop but the dialog will not minimize. it will stay on the screen.
when you use windows d everything is minimized. includeing that dialog box.

I was a Window Eyes user. With W.E., when leaving a program I was returned to the desktop. With Jaws, I have to hit windows-D to get there and sometimes have to hit it twice. to return to desktop. Is there something I can change to get directly to desktop, upone closing a program?
