          Yes, it runs in the background and there is an icon in the system tray (which is usually hidden on Windows 8 or Windows 10 unless you tell it to always show) for Unchecky.  It runs as a system service, not like a regular program, and if you bring up the control pane via that icon the button that allows you to shut it down reads, "Suspend Service" not "Exit."

           It pretty much has to run constantly in the background if it's to perform its intended duty.  Of course, it doesn't have to if you are diligent enough to activate it before each install, but most people aren't, and it's designed to keep most people from ending up with stuff they don't want on their systems.
