hello Russell,
You could start by demonstrating how fiendishly confusing the main Youtube interface is. It is true that we have the accessible youtube page ( at http://tube.majestyc.net/?s=skip+to+the+good+bit )
but the latter doesn't seem to be fully functional all the time. for example, sometimes the search results display, no matter how many times you refresh and redraw the thing.
Apologies if this isn't exactly what you're looking for, it nevertheless should be a good example of how not to design a website for optimum screen reader access.
