You've seen the discussion on how to open or switch focus to programs in Win7 using the Windows key + numbers from 1 to 0. If you add the shift key to any of these commands, it will open a second instance of a program. Could something like that explain it?

Have you first tried to close one of those windows to see if the other remains? Happens to me on very rare occasions, and I have no idea why.
